NSSAR Copy Request Form (no form number, last revised 2001 Feb) is used to request that SAR headquarters staff do ONE of the following
-- a) search for and send you the national numbers of SAR applications which name a specific patriot ancestor
-- b) send you a copy of a the application for a specific member by name or by national number

Forms for Change in Membership Status

Transfer of Membership (Form 0919: last revised 2000 Mar) -- Use this if you wish to change your primary affiliation to a new state.

Member Transmittal Form (Form 0921, last revised 2001 Feb) -- State secretaries and others use this form to accompany materials dealing with member status. This version has no form fields. Please print this form on canary yellow paper so it won't get lost in the documentation that it accompanies. You may then either fill it in with a typewriter or overprint the form with a word processor page which is laid out to start 4.25" down the page with 0.75" margins and tabs set at 1.25, 2.5, 5.25, and 6.25".
